Age Requirements for Admission
- Child must be 3 or 4 on or before August 1 for Child Care/Preschool
- Kindergarten students must be 5 years of age by August 1.

New Students
Starting the Admissions Process
We are currently accepting registration information for new students. Applications for admission can be downloaded from our website or can be picked up in the school office. Kindergarten through grade 8 students new to John Paul II Catholic School must submit the following documents:
Baptismal Certificate if Catholic
Birth Certificate
Social Security Card
Physical Form
Kentucky Eye Examination Form
Kentucky Dental Screening Form
Immunization Certificate
$275 Registration/Book Fee
Returning Students
Students returning to John Paul II Catholic School must submit:
- 2023-24 Tuition Contract
- Updated demographic information completed in Fast Direct
- $275 Registration/Book Fee
Sixth grade students must submit an updated physical and immunization form for entrance in the 6th grade.
Physical Form
Preschool/Child Care Students
Preschool/Child Care students must submit:
- Updated Immunization Certificate
- Baptismal Certificate (if Catholic)
- Birth Certificate
- Social Security Card
- $50 Registration Fee (pre-school only)
- Kentucky School physical, completed after March 1, 2023
